You'll also have to do a little bit of very simple math to use it. I'm not handing the modifiers of every item out.
It's very similar to drop items only instead of changing items that you physically throw out into the world, you're changing the effects of items.
For example: Everyone hates bloopers, right? Such a useless item filled with fail. Wish it turned you huge like a mega shroom does? This is the code for that.

Fun Fact - You can make what I like to call "combo items" with this code. That's when you give an effect to an item that normally doesn't have one. For example: Blue shells have the effect of causing them to be undraggable (see disable holding effect). Well, if you change that with this code, not only will you throw the blue shell, but you'll also activate whatever effect you put on it. So, you could have a blue shell/lightning combo. Can you say, "Rob someone of their mushroom so they can't do a blue shell dodge without the need of lightning"?
